Lifestyle Change Program
The Pathway of Health NPP Lifestyle Change Program is a year-long, evidence-based program that helps people at risk reduce their chances of developing type 2 diabetes by 58% through healthier eating, increased physical activity, and behavior changes. It includes weekly group sessions led by trained coaches, focusing on sustainable lifestyle habits that improve health and prevent diabetes. The program encourages weight loss, stress management, and ongoing support to maintain long-term results.
12 Month Program – DPP/Prevent T2 (diabetes) Lifestyle Change
The CDC-led National Diabetes Prevention Program helps people with pre-diabetes make lasting lifestyle changes to prevent or delay type 2 diabetes.
What is prediabetes? Your blood sugar is higher than normal but it is not high enough to be diagnosed with diabetes (type 2 diabetes).
If you have prediabetes, you are more likely to:
- Get diabetes
- Get heart disease
- Experience a stroke

Working with a trained Lifestyle Coach in a CDC recognized organization, you’ll learn to eat healthy, add physical activity into your life, and manage stress. With other participants, you’ll celebrate successes and work to overcome challenges. Pathway of Health follows the strict curriculum of the evidence-based CDC program of lifestyle change for anyone who is at risk with pre-diabetes. You must be 18 and not pregnant to qualify for this program and you cannot have been diagnosed of diabetes. You will be attending support group meetings every week for the first six months of this program and then at least once a month (depending on your group’s preference) for the last 6 months.
